Steps to Reporting

Are you looking for resources when you feel you have been discriminated against or have a DEI issue or complaint? Please see below for resources for students, faculty, and staff for reporting these.

Student Resources

  • CARE Referral form: The Campus Assessment, Response, and Evaluation Team (CARE Team) engages in various approaches to identify and assess students who are distressed or may exhibit concerning behaviors. Faculty, staff, and students should use this form to notify the CARE Team about any student concerns. This could be a wide variety of things: from a student who stopped going to class to a student experiencing suicide ideation.
  • Personal Misconduct: the Office of Student Conduct and Care is responsible for upholding the Code of Student Conduct. Faculty, staff, and students are encouraged to submit incidents they encounter via the link below. This addresses only student behavior.
  • The Office of Student Conduct and Care assists students if they have a complaint against a faculty member within the context of the classroom (grade appeals, other general academic issues). The director walks students through their options to report formally and informally. Students can call or email with questions.

Faculty and Staff Resources

  • PFW is committed to maintaining an inclusive community that recognizes and values the inherent worth and dignity of every person; fosters tolerance, sensitivity, understanding and mutual respect among its members; and encourages individuals to strive to reach their own potential. In pursuit of its goal of academic excellence, PFW seeks to develop and nurture its diversity. PFW does not condone and will not tolerate Discrimination or Harassment against any individual on the basis of race, religion, color, sex, age, national origin or ancestry, genetic information, disability, status as a veteran, marital status, parental status, sexual orientation, gender identity or gender expression. All members of the PFW community are encouraged to report any incident of Discrimination or Harassment.

  • All formal processes for allegations of discrimination or harassment on any basis are governed by the system-wide EEO policies and procedures implemented by the Vice President for Ethics and Compliance Office.

  • Matters that meet specific jurisdictional elements of Title IX are governed by these policies and procedures.

We encourage reporting of any incident, no matter the basis or the parties involved, through our online conduct software Maxient, which can be found through HR-OIE, through the Office of Student Conduct and Careor reports can be made via the Whistleblower Hotline.
